In the first, she is disguised in a nun's habit; she picks up a small cigar that Eastwood had thrown away, and quadruple-pumps it, followed by a long exhale and a sigh of relief. In the second, (Eastwood has learned she's really a prostitute) she plucks a cigar out of a man's mouth and smokes it.
